Fees For Services

Private pay fees are:

$160 for an intake session

$150 per regular session.

Full session fee is charged for late cancellation/no show fees

Sliding scale fees are available on a case-by-case basis.

*Insurance rates may differ from private/cash pay rates.

You will be required to have a card on file* and your session fee is due the day of the service. We will run your card for your session fees or copays at the end of your session.

Your appointment time has been set aside for you specifically. If you need to reschedule your appointment time, please notify us as soon as possible. If you cancel your appointment less than 24 hours prior to the scheduled time, or you do not show up for your appointment, you will be assessed the full session fee. A full service fee will be assessed under the following circumstances: “forgetting” you have an appointment, conflicting appointments, or choosing to complete other important tasks rather than keep your appointment.

Reminders for appointments are a COURTESY. You are ultimately responsible for your appointment times. You are able to check on your scheduled time by logging into your client portal.

Emergencies do happen, but these are circumstances outside of your control such as natural disasters, car accidents, hospitalization or urgent care visits, or similar circumstances. I will not charge for a true emergency.

If your credit card is declined, you will have 5 business days to clear the balance before you are placed on a scheduling hold. You may log into your client portal at any time to manage your payment options. Any remaining balances that are left unpaid may be subject to collection attempts.


Insurance

HM Counseling Services will file your insurance claim for you and collect you co-pay/co-insurance the day of your session. If your insurance company denies your claim after 30 days for any reason, you will be responsible for paying the private pay fee for your clinician. We will make attempts to investigate the denial issue with your insurance company, but it is ultimately your responsibility to pay your bill. Should your insurance pay the bill at a later date, you will be reimbursed immediately.

It is your responsibility to know if you require a referral from your insurance to see me for services. If a referral is required and not obtained prior to services, they will not pay and you will be responsible for the full fee.

We accept Anthem BCBS, Cigna/Evernorth, Aetna, Optum/United, all Medicaid RAEs, and CHP+ for Colorado Access and Rocky Mountain Health Plans


Paperwork

Paperwork, to include, but not limited to, reading and responding to e-mails, letters of verification to probation officers, Child Services, etc., and/or other documentation will be assessed a $30 fee for each 15 minute increment required to fill out documentation.

Please be aware, we will NOT complete FMLA, short or long-term disability, Social Security, and we do not write letters for Emotional Support Animals. These letters will need to be completed by your medical doctor or prescriber.
